Benefits at a Glance

i2 offers the following benefits. Most of the employee’s cost for coverage is paid by i2:

Employees may cover their spouse or same-sex domestic partner and eligible children.

Health Care Benefits

Medical
Three medical plan choices cover routine and emergency care, hospitalization and surgery, preventive care, prescription drugs, and mental health and substance abuse treatment:

  • United Healthcare PPO offers broad range of coverage, and large network.

    Plan A - $350 deductible, 80/20 coinsurance

    Plan B - $250 deductible, 90/10 coinsurance

    Plan C - $0 deductible, 0/100 coinsurance

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: i2 pays most of the cost for employee and dependent coverage (including same and opposite sex domestic partners) under all three UHC PPO plans. Employees pay a lesser portion of the costs through regular paycheck deductions.

Employee Assistance Plan (EAP)
This plan provides confidential telephone counseling and referrals for personal problems such as depression, anxiety, grief, and substance abuse. Employees can receive up to six free counseling sessions with a licensed counselor per person, per issue. In addition, EAP offers convenience services, such as search for child and elderly care, house keepers, etc.

Eligibility: Available to all i2 full-time regular employees.
Cost: Paid in full by i2 Technologies.

Dental
The i2 dental plan allows employees to select any dentist when dental care is needed. Covers a significant portion of the cost of dental treatments:

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: i2 pays most of the cost for employee and dependent coverage (including same-sex domestic partners) under the UHC Dental Plan. Employees pay a lesser portion of the costs through regular paycheck deductions.

Vision
This plan provides a $200 annual allowance per covered person. Benefits cover eye exams, lenses, frames and contact lenses.

Eligibility: Available to employees and covered dependents enrolled in any i2 medical plan.
Cost: Paid in full by i2 Technologies.

Spending Accounts
i2 offers a Health Care Spending Account and a Dependent Care Spending Account to set aside tax-free money to be reimbursed for eligible expenses.

  • Health Care Spending Account: Employees can contribute up to $3,000 pre-tax for reimbursement of eligible medical, dental and vision expenses not covered by insurance.
  • Dependent Care Spending Account: Employees can contribute up to $5,000 pre-tax for reimbursement of qualified dependent care expenses.

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: Employee elects payroll deduction amount.

Income Protection Benefits

Basic Life Insurance
i2 provides basic life insurance coverage for the employee and the employee's eligible dependents:

  • For the employee: One times base salary (up to $400,000).
  • For the employee’s covered dependents: $5,000 for spouse or same-sex domestic partner and $2,500 for each child.

Eligibility: All full-time, regular employees are eligible from their first day of work.
Cost: Paid in full by i2 Technologies.

Supplemental Life Insurance
Additional life insurance coverage of one-to-five times base annual salary (up to $750,000) is available for employees. Employees who elect Supplemental Life Insurance for themselves may also purchase Supplemental Dependent Life Insurance. For a spouse or same-sex domestic partner, coverage is available in $10,000 increments up to $250,000, but may not be more than 50% of the employee coverage amount. For children, coverage of $5,000, $15,000, or $25,000 is available.

Eligibility: All full-time regular employees are eligible from their first day of work. Evidence of insurability may be required.
Cost: Paid in full by the employee.

Basic Accidental Death & Dismemberment (AD&D)
AD&D Insurance pays a benefit if an employee dies or is injured in an accident. The coverage amount is the same as for basic life insurance.

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: Paid in full by i2 Technologies.

Supplemental AD&D
Additional AD&D coverage of one to five times base annual salary (up to $500,000) is available for employees. Employees who elect Supplemental AD&D for themselves may also purchase AD&D coverage for their dependents. The maximum coverage amount for dependents is a percentage of the employee coverage amount:

  • Spouse only: 60% of employee Supplemental AD&D coverage.
  • Spouse and children: 50% of employee Supplemental AD&D coverage for spouse and 10% for each  child (up to a maximum of $25,000 per child).
  • Children only: 15% of the employee's Supplemental AD&D coverage for each child (up to a maximum of $25,000 per child).

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: Paid in full by the employee.

Short-term Disability (STD)
STD coverage pays 100% of the employee’s base salary for up to 12 weeks if an employee cannot work because of an illness or injury.

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: Paid in full by i2 Technologies.

Long-term Disability (LTD)
LTD coverage pays 60% of the employee's eligible earnings, up to $10,000 per month, if the employee cannot work because of illness or injury.

Eligibility: All full-time regular employees are eligible from their first day of work.
Cost: Paid in full by i2 Technologies.

Wealth Accumulation Benefits

To help employees prepare for future financial needs, i2 offers two valuable benefits:

401(k) Savings Plan

  • Employees can elect pre-tax contributions of 1% to 25% of eligible pay, up to the legal limit
  • Offers 18 investment options plus a self-directed brokerage window
  • Plan loans are available
  • Hardship withdrawals are allowed in certain situations

Eligibility: Employees at least 21 years old are eligible to enroll after one month of service.
Cost: Employee elects the percentage to be deducted from pay.

Employee Stock Purchase Plan (ESPP)
Employees may contribute 1% to 15% of base pay to purchase i2 Technologies stock. Employees buy stock at a 15% discount and hold it or sell it at the current market price.

Eligibility: Regular employees working at least 20 hours per week are eligible for enrollment each November 1 and May 1.
Cost: Employee elects payroll deduction amount.

Paid Time Off

i2 offers paid time off for vacations, holidays, jury duty, bereavement leave, paternity leave, adoption leave, and sick leave. The typical vacation accrual schedule for full-time employees is:

  • 1 year of service or less: Accrual of 1 day per month up to 12 days annually.
  • 2-4 years of service: Accrual of 1.25 days per month up to 15 days annually.
  • 5 or more years of service: Accrual of 1.67 day per month up to 20 days annually.

Eligibility: Full-time regular employees who work at least 30 hours are eligible for the above accrual schedule. Part-time employees earn vacation at half the full-time accrual schedule.
Cost: Paid in full by i2 Technologies.

Volunteer Benefits

i2 offers volunteer benefits for legal services, identity theft, and pet insurance.

  • Pre-Paid Legal Services offers a broad range of legal counseling and representation for a low monthly fee. This benefit is transferable.
  • Identity Theft Protection provides you with certain protections against identity violators and will restore your identity with the help of an assigned legal counsel.
  • Pet Insurance is our way of protecting our favorite pets from different sicknesses ranging from preventive care and ending with cancer treatment.
